After Hours Pediatrics Urgent Care needs your help to create our holiday greeting card. The winner's masterpiece will be featured on our holiday card that will be sent to pediatricians throughout Tampa and South Florida. The winner's family will also receive 50 holiday cards for their own use.

Instructions: 1. Contest is open to children ages 5-12. Entries will be accepted Sept. 1, 2016 through Oct. 31, 2016. 2. Open and print the entry form by clicking on "Contest Entry Form" below. Draw entry within the purple border to illustrate our theme, "A Florida Winter Wonderland." 3. Submit drawing in color by scanning and emailing to info@afterhourspediatrics.com, or mail to Attn: Holiday Card Drawing Contest, 4035 Crescent Park Drive, Riverview, FL 33578. 4. Provide the requested contact information on the entry form. |
